Our office protocols when you visit our office:
1) For each eye exam, we will email Health History form via Adobe Acrobat. Please fill out Health History with updated contact and insurance information prior to eye exam. If unable to open Adobe, we can email a PDF copy or patient can fill out in-office..
2) All contact lens orders will be shipped directly to patients, unless requested for in-office pick-up.
3) We will prioritize assisting Eye Exam and Optical Browsing appointments over walk-in browsing.
